Continuum of Care Campus
Momentum Healthcare LLC offers a continuum of care campus in Franklin Grove, as well as other locations (Oregon and Shabbona.) Franklin Grove Living and Rehabilitation Center is located on the same campus as our Assisted Living Community. During the admissions process, our knowledgeable staff will assist you in finding the appropriate placement for you or your loved one. Whether you choose The Meadows of Franklin Grove Assisted Living Community as your new home or are more suitable for our skilled nursing facility next door, our goal is to provide excellent service. From our Independent/Assisted Living apartments, The Meadows, to our intermediate, skilled and Medicare settings, every level you need can be found on one campus.
Our sister facilities are located in Oregon and Shabbona. Oregon Living and Rehabilitation Center is located in the peaceful town of Oregon. Residents and guests are treated with respect and dignity during their stay. This facility offers 24/7 skilled nursing care, long-term care, respite, hospice and a secured memory care unit. Our experienced staff are here to make sure you are well taken care of. Nearby in DeKalb County is our other Continuum of Care Campus.
The Prairie Crossing facilities are located just outside of Shabbona Lake State Park. Prairie Crossing Supportive Living Community is suited for those individuals who would like to maintain their independent lifestyle without the burden of maintaining a home. They offer apartment style housing for those individuals aged 65+ years. If you or your loved one require more care, the other side of our campus maybe better suited for you.
